NHS England has published its 2024/25 Type 2 Diabetes Local Data Packs, providing practices and Primary Care Networks (PCNs) with valuable insight into local population need, diabetes prevalence and care delivery.
The packs are designed to help primary care teams identify variation, target quality improvement activity and plan services more effectively to improve outcomes for people living with type 2 diabetes.
Why this matters for general practice
This year’s data packs come at an important time, as delivery of all eight NICE-recommended diabetes care processes is now a key requirement within the Quality and Outcomes Framework (QOF).
Practices should be aware that patients must receive all eight diabetes care processes for the practice to achieve the relevant payment. This makes it more important than ever to have robust recall systems in place and to identify patients who are missing one or more of the required care processes before the end of the reporting period.
The eight care processes are:
- HbA1c measurement
- Blood pressure measurement
- Cholesterol measurement
- Serum creatinine test
- Urinary albumin test
- Body Mass Index (BMI) measurement
- Smoking status review
- Foot examination
The new Local Data Packs can help practices identify gaps in care, benchmark performance against local populations and target patients who require review, supporting both improved patient outcomes and QOF achievement.
Accessing the data packs
Primary care teams can access the Type 2 Diabetes Local Data Packs via the Future NHS National Diabetes Programme webpage (NHS login required).
Updated versions of the packs will be published in the autumn.
